| To open up the above National Application, click on the above link and
it will open up in Adobe Reader. Most Computers come with this
preinstalled but if your's don't have it or you have deleted this program
then it is available for free at Adobe (www.adobe.com). Click on the below
image to get Adobe Reader. |

Once the application opens up:
- Click 'File' (in the upper left hand corner of the window.
- Select 'Print'.
- Print the application out on your computer.

| When the application is printed out (on your own printer), fill it out
and send the completed application (with the supporting documents, copies
will do) to the address of the National Adjutant listed at the bottom of
the application. |
